Insurance and Medicare
We will file a claim to your primary insurance carrier and secondary carrier, if applicable, when the information has been provided. You should notify the office immediately regarding any changes in your insurance carrier or plan.
Managed Care
Managed care plans have strict requirements. Please verify the physician's participation and authorization requirements with your managed care plan prior to your visit by contacting your insurance's customer service department. It is impossible to know every insurance plan for every patient; therefore, it is your responsibility to know of any restrictions or requirements for your plan. Failure to receive correct verifications or authorization may result in insurance denying coverage, in which case, you will be responsible for payment of services rendered.
Medicare
Dr. Moore and Dr. Chitale participate in the Medicare program. Medicare pays our providers directly 80% of the approved charges. The patient is responsible for payment of the annual deductible and the 20% co-payment as determined by Medicare.
Commercial Insurance
We participate in a variety of commercial and workers' compensation programs and networks.
New Patient Information
If you are visiting us for the first time, please arrive 15 minutes before your scheduled appointment to allow time for registration. Please bring your medical insurance card(s), a list of medications you are currently taking and any forms mailed to you by our office, x-rays, scans, and other pertinent information.

Financial Policies
Our trained patient account representatives are here to assist you with any of your billing, insurance and payment questions or concerns. If you have questions, please call our business office at 770.424.2025.
While your health is our first priority, we are sensitive to the high cost of health care and will do our best to deliver care in the most cost-efficient manner possible. Thus, we ask you to make payment for office services at the time of your visit. This avoids the high cost of billing and collection of past-due accounts. Our office honors MasterCard and Visa for your charge convenience.
Prescriptions and Refills
If possible, please call at least three full days before your prescription will deplete, allowing our staff the opportunity to collaborate with your provider. When calling for a prescription refill, please give your name, birthday, daytime phone, the name of the medication and the name and phone number of the pharmacy you utilize.
If you request a refill after business hours or on weekends, your pharmacy will be contacted within two business days. When you are given medication, try to finish the prescription. If you are experiencing side effects or you feel your condition has not improved, call the office or communicate this to your provider on your office visit.
